feat!: add connection type column to _pico_peer_address and store pg_proto address in _pico_peer_address

Close #1114

Instance info is cluster-wide information. You never need to call
.instance_info on every single instance, they all have the same
information. You only need to connect to a single instance to get all
the info about the cluster (except for some runtime information, which
is the rare case).

This is a breaking change, because the default behavior has changed, an
online instance will not be expelled with just `picodata expel`.
